Features an authentication label in the collar bearing the certification number "1N13659, " from the NFL and PSA/DNA. The white mesh jersey is numbered "54" on the front, reverse, and each sleeve. "Urlacher" is lettered upon a nameplate on the reverse. All letters and numerals are sewn in navy on orange tackle twill. The initials "GSH, " in honor of former Bears owner and coach George Halas, appear on the left sleeve. Located on the left front tail are a "Reebok" label and adjacent "50" size tag.
